Purpose and Basic Concepts of Urodynamic Study

Purposes of Urodynamic Investigation:

  • To evaluate detrusor and sphincter function/dysfunction
  • To classify the nature of voiding and storage dysfunction
  • To identify urodynamic risk factors for the development of upper urinary tract deterioration.
  • To determine the precise etiology of the patient’s incontinence
  • To optimize treatment plan to achieve balanced voiding in SCI patients: to determine the need for correction of anatomic abnormalities

Basic Urodynamic study

  • Evaluation of bladder storage function
    • Cystometry (cystometrogram, CMG)
    • Bladder outlet
    • Leak point pressure
    • Detrusor leak point pressure
    • Valsalva leak point pressure
    • Urethral pressure profile
  • Evaluation of voiding function
    • Uroflowmetry
    • Pressure-flow stud

Electrophysiologic tests

  • Kinesiologic studies: EMG
  • Neurophysiologic recordings
  • Nerve conduction studies

Key Points of This Section
  • There are many ways to evaluate lower urinary tract function in neurogenic populations.
  • Urodynamics in neurogenic conditions are relatively straight forward.
  • These observations are directed at the risk factor in lower urinary tract function.
